Act 151 Child Abuse History Clearance

All prospective employees must submit an official copy of this form prior to employment.  This clearance must be less than sixty months, five years old from the employee's date of hire.  Volunteers may obtain this clearance free of charge.  Please note that a volunteer clearance from a paid employee will not be accepted.  This clearance must be renewed every sixty months based on the date of the clearance.
