Bloomsburg Area Middle School

   

   Physical Examinations:  Required for all students in 6th and 11th grades.

 

   Dental Examinations:  Required for all students in 7th grade.

 

 

   BMI (Body Mass Index): Completed each year on all students (6th-12th grades).  Results will be sent home to parents/guardians.

 

   Vision:  Completed each year on all students (6th- 12th grades).  Referral letters will be sent home for any students who fail the vision screening.

 

   Hearing: Completed on 7th and 11th grade students.  Referral letters will be sent home for any students who fail the hearing screening.

 

   Scoliosis:  Completed on students in 6th and 7th grades.  Referral letters will be sent home for any students who fail the scoliosis screening.

 

 

*All 6th grade students are also required to have two vaccinations: Tdap and MCV.  Most of the students will receive these at their private physical examinations.  These will not be given at the school physicals.  Parents will be notified throughout the year if we still do not have documentation of your child's required immunizations.  Both are required for students to start 7th grade.  Immunizations can be received at your physician's office or you can see if you are eligible to receive them from the Department of Health.
