We recommend that students take medication before or after school, however we understand that this is not always possible. When a student must take medication during school hours, we require a note from the parent AND a doctor's order. As nurses, we are unable to give any medication without a doctor's order. Below is a form that can be filled out by your child's physician to ensure there is no delay in your child getting their medication during school hours.A parent or guardian MUST bring the medication in the school to the nurse's office (located in the Middle School--please come to the Middle School Front Office to bring any medication for students).
Medication Administration Request Form
Medications that only require a parent note are: Acetaminophen (Tylenol), Ibuprofen, Antacids, Cold/Cough/Allergy Remedies/Decongestants, Midol, and Imodium. Any other medication must have the above form completed by a physician!
